    • A copy of your Proof of Income. Below are the acceptable documents:
      • BIR Form 2316 (W-2) for employed/fixed salary earners
      • BIR Form 1701/1702 with BIR or bank stamp for self-employed
      • Last three months payslip
      • Latest credit card statement

    1. UnionBank of the Philippines to transfer and disclose to any Credit Institutions such as financial institution, credit bureau, banking or credit industry associations, credit information service provider, credit and loan providers and their respective outsourced service providers; and
    2. any such Credit Institutions to transfer and disclose to any fellow user, member, subscriber, whether a financial or non-financial institution, or to such Credit Institution’s outsourced service provider, any information (updates or corrections), whether positive or negative, relating to me/us or any of my/our accounts with UnionBank of the Philippines (and for such purposes) for the duration of and even after the closure or cancellation of the Account/s.
