Application Requirements

To process your credit card application, please submit the following requirements via:

  • Mail to 31/F UnionBank Plaza Meralco Avenue corner Onyx and Sapphire Sts., Ortigas Center, Pasig City – Attention: Credit Card Acquisition Department

1.Copy of two (2) Valid IDs with photo and signature plus three (3) specimen signature

2.Proof of Income

  1. If employed/ fixed salary earner: BIR form 2316(W-2)
  2. If self-employed:
    • Income Tax Return BIR form 1701 or 1702 with BIR or bank stamp
    • Department of Trade and Industry Certificate of Business Name Registration

3.Proof of Billing Address (IF OUTSIDE METRO MANILA) – latest statement

a. UnionBank of the Philippines to transfer and disclose to any Credit Institutions such as financial institution, credit bureau, banking or credit industry associations, credit information service provider, credit and loan providers and their respective outsourced service providers; and 

b. any such Credit Institutions to transfer and disclose to any fellow user, member, subscriber, whether a financial or non-financial institution, or to such Credit Institution’s outsourced service provider.
