Requirements

These Standard Forms need to be accomplished to invest in our Funds:

For Individual Accounts:

1. Participating Trust Agreement (PTA)*
2. Risk Disclosure Statement*
3. Client Suitability Assessment (CSA)*
4. Signature Card
5. Photocopies of two (2) IDs of the signatories, TIN Nos., and SSS No.

For Corporate Accounts:

1. Participating Trust Agreement (PTA)*
2. Risk Disclosure Statement*
3. Client Suitability Assessment (CSA)*
4. Signature Card
5. Photocopies of two (2) IDs of the signatories, TIN Nos., and SSS No.
6. Articles of Incorporation and By Laws (Certified True Copy),
7. Board Resolution appointing UBP-Trust & Investment Services Group as the Trustee and signatories of the account (Original Copy).

* Documentary requirements for each Fund would vary.

Upon completion of all requirements, send to the nearest UnionBank branch.
